The Situation
On September 30, 2007, the Dept. of Health and Human Services, Centers for Medicare and Medicaid Services awarded to The State of Florida, Agency for Persons with Disabilities (APD) a $500,000 Real Choice Systems Change: Person-Centered Planning Implementation Grant. The term of the grant extends from October 1, 2007 through September 30, 2010.
APD’s grant was based on a partnership with CQL for state-wide training, consultation and technical assistance. CQL’s grant-related activities will support APD’s continued quality management and improvement strategies through system change and internal capacity-development. Focus will be given to continued education and training of APD’s Area Quality Leaders on CQL’s Community Life ® strategy to define, measure and develop quality enhancement activities around community issues.
CQL’s Community Life ® LENS directly supports meeting mandatory components of the grant listed as:
1) an informal community network assessment and intervention process for the consumer to create enduring and meaningful ties to organizations in her/his community
2) a proposed evaluation of quantifiable outcomes (CQL’s Personal Outcome Measures ® data collection and Community Life ® indicators and factors data analysis and synthesis)
What CQL Will Do
CQL will lead APD’s Area Quality Leaders through CQL’s Community Life ® LENS to explore quality of life within a community. What makes this exercise unique is that it isn’t just talking about good ideas – it’s real action and change –inspiring people to make something happen. Area Quality Leaders will go out into the community and investigate education, employment, health care, transportation, housing, and recreation and leisure. Then they develop real plans to address community issues that impact all people.
